Feedback request for dial up service

We are asking for a "little" feedback from our dial up customers about the service and support they are getting.
We do not want to be swamped with junk emails please but if you have a good thing to say or even a bad thing to say (constructive criticism please) send us an email.
We want to know how we are doing and welcome suggestions about improvements or just a note from people to say hello.
